Application Deadline February 10, 2012

The essential information will be drawn from the OSU Student Information System. Be sure that all of your records are up to date in the university’s student information system. You must have been accepted for admission by March 1, 2012 to be eligible for consideration of all scholarships. To be eligible for scholarships requiring evidence of financial need, a FASFA form must be on file with the Financial Aid Office. This application covers all the College’s scholarships. NOTE: If you wish to be considered for the Art Koski Travel Grant you must also complete the additional form which is available at CPHHS Undergraduate Scholarships

Submission Requirements

  • *Statement of Professional Goals - Present your professional goals, challenges you face in earning your degree, commitment to 4-H and/or Extension, (if applicable), and related information about yourself as a scholarship recipient in one-double spaced, typed page.
  • *Resume with current mailing address, email and phone noted (limited to two pages)
    Organize your resume as follows:
    1. Education
    2. Work experiences
    3. Organizations and activities, include membership in Kappa Omicron Nu, and current/past resident of Azalea House if appropriate
    4. Community service
    5. Honors and awards
    6. 4-H and Extension/Outreach Experience, include name of club, county and number of years involved
  • *Transcripts – Unofficial transcripts are acceptable, from both OSU and transferred course work

To be eligible to receive a scholarship administered by this College for the 2012-2013 academic year a student must be enrolled full-time in the College of Public Health and Human Sciences throughout the Fall, Winter and Spring terms of the 2012-2013 academic year. A student who graduates during any of these terms shall receive award distributions only for the terms enrolled. Awards are divided evenly among three terms (Fall, Winter and Spring), unless the recipient is notified otherwise. Funds are credited to the student’s Oregon State University account. Scholarship applications must be submitted prior to 5:00 p.m., February 10, 2012. Applicants shall be notified of the outcome of the consideration of their applications, by mail, at their current address by April 6, 2012.
