Procedures
Nomination procedures entail submitting a concise statement, not exceeding 500 words, outlining the nominee's specific contributions for the award consideration. (See award categories)
Additionally, the nominee's resume or CV should be included, if applicable.
Deadline
All materials must be submitted through the nomination form by 5 p.m. May 1.

Categories
The COH Professional Faculty Excellence Award recognizes a professional faculty member (unclassified) who has achieved distinction in their profession, unit, COH, and OSU, and who epitomizes excellence in their activities.
The COH Staff Excellence Award recognizes a staff member (classified) who has achieved distinction in their profession, unit, COH, and OSU, and who epitomizes excellence in their activities.
The COH Extension & Engagement Excellence Award recognizes any staff or faculty member (academic or professional) or team who has achieved distinction in their profession, community, unit, COH, and OSU, and who epitomizes excellence in their engaged activities.
The COH Service Excellence Award recognizes any member of the COH who has achieved distinction in service to their profession/field of discipline, community, unit, COH, and OSU, and who epitomizes excellence in their service activities.
The COH Research & Scholarship Excellence Award recognizes a faculty member who has achieved distinction in their profession/field and who epitomizes excellence in their research and scholarship activities.
The COH Junior Faculty Excellence Award recognizes any faculty member who has not reached promotion and who epitomizes excellence in their assigned activities.
The COH Teaching & Mentoring Excellence Award recognizes instructional faculty who have achieved distinction in teaching (for credit or noncredit) and mentoring related to their discipline and who epitomizes excellence in their teaching and/or mentoring activities.
The COH DEI Excellence Award recognizes any COH staff or faculty member who actively promotes diversity, equity, and inclusion through consistent and sustained efforts to improve opportunities for the diverse communities we serve.
The COH Creative Activity Excellence Award honors outstanding achievements by members of the College of Health in the realm of creative endeavors. This award is presented to a COH student, staff, faculty member or team who has demonstrated excellence in creative activities, spanning visual arts, music, dance, and beyond.
Dean nominated
The COH Dean’s Unsung Hero Award recognizes an exceptional individual or team within the COH who, despite working behind the scenes, has made invaluable contributions to the success and well-being of the college. This award celebrates the often unnoticed yet crucial efforts of an unsung hero, highlighting their dedication and impact within the academic environment.
Based on date of hire (not nominated)
The Length of Service Award honors individuals for their dedicated service and commitment to the COH in 10-year increments. Awardees are provided with a certificate, no monetary award.
Award
The awardee will receive a certificate of recognition along with a $200 monetary prize at the fall college meeting.
Their name, along with notable quotes from the nomination, will be prominently displayed in the Dean's Suite hallway throughout the academic year.
While the standard practice is to bestow one award annually for each category, in the event a suitable candidate cannot be identified, the award will not be granted.
Eligibility
Staff and faculty members are eligible to receive more than one award per year.
Award selection committee
In May of each year, a committee comprised of past awardees will convene with the ADAFA to review nominations and subsequently recommend candidates to the Dean and Associate Deans for the final determination of the award recipient. To serve on the review committee, a member would not be eligible to nominate an individual for an award, so additional committee members may be sought in order to have a group to conduct the review.
