Materials

All CPHHS graduate programs use SOPHAS for admissions processing. You must submit all materials directly to SOPHAS.

Transcripts

Do NOT send official transcripts to Oregon State University. SOPHAS requires you to send ALL official transcripts to:

If you have taken ANY courses at an institution, you are required to submit the transcript from that institution (including Oregon State University); otherwise SOPHAS will not be able to verify your application.

The verification process can take quite a while, so you should submit your transcripts to SOPHAS as soon as possible (as soon as you begin your application, you should have your transcripts sent to SOPHAS). If you wait until the deadline to request your transcripts, it is extremely unlikely that your application will be considered for that deadline. Your application cannot be reviewed by the committee until all transcripts are verified.

Official GRE scores

Your official GRE scores should be sent directly to SOPHAS (using code 0031). Do not send the scores directly to OSU. You must take the GRE prior to the deadline for which you are applying.

Fee waivers

SOPHAS offers various fee waivers for participants in particular programs. You can check to see if you qualify for a fee waiver at SOPHAS Specialized Fee Waivers

If you have completed a degree in a country other than the U.S. or Canada

Students who have completed a degree in a foreign country (other than Canada): you will need to submit a foreign evaluation through World Education Services. This process can take weeks depending on how quickly your foreign institution sends official transcripts. For more information, please see International Credential Verification Reports for SOPHAS

Once you submit the order, be sure to designate SOPHAS as a recipient of the results. SOPHAS only loads foreign evaluations once a week, so you will want to submit this request as soon as possible in order to meet the deadline for which you are applying.

If you have participated in a study abroad program as part of your undergraduate program, you do NOT need to complete a foreign evaluation.
